Standard Hours: 8am-6pm, Monday-Friday. 
Each of our therapists have flexible hours to meet clients needs.

Fees

Hourly Rate: $95 per 50 minute session

Sliding Scale: A reduced fee is available to those who qualify.  Please contact the particular therapist to discuss.

Student Discount:  We gladly offer 20% off our sessions for those who are in college.  Please be sure to let us know if you are in school and have your student ID with you.

Forms of Payment: Visa, MasterCard, Discover, American Express, Cash, Check, and Flex Cards for insurance.

Insurance:  Each of our therapists are considered "Out of Network" - call your health care provider when you schedule your first appointment.  An invoice can be provided to you for your records.  (Please see our Frequently Asked Questions for an explaination of why we choose to be "Out of Network.").

Forms

Below are some forms you can print out and fill out before attending your first scheduled appointment.  Everyone will fill out the "Client Demographic Forms."  If you are bringing a child in for therapy, please fill out the "Parental Consent to Treat a Child."  Additionally, if you and your child's parent are separated, please have the other parent sign the "Other Parent Consent."

We have also attached the "Description of Counseling and Confidentiality" form as well as the "Notice of Privacy Practices" form for your convenience.  They will also be made available to you during your assessment.
